Overview
This article explains how to customize forms that are used for creating learning experiences. Specifically, this article covers how to attach Foundry Assessments to learning experiences. It is assumed you have already reviewed the articles on how to Create a Form, work with Required Prompts, Basic Prompts, and Intermediate Prompts. If you have not created an Assessment, review the process or search your forms for an existing Assessment if any were pre-loaded into your Foundry site.

Align Assessment
This option allows the experience designer to select an existing assessment for students at the end of the experience.

Scheduled Assessment
When you schedule an assessment for a student experience, Foundry will prompt the student about the upcoming due date.

Pre-selected Assessment
As the text in the below image states, this option requires some planning. It means every time this experience form is used, the assessment you select when designing it will be a part of the experience.
To pre-select an assessment, you will need the assessment ID before adding it to the form.
It is found at the top of the screen when you open the form editing screen:
You'll need to copy that ID and enter it into the 'Option Text' box in form editing mode (the ID's shown above and below are for different assessments):

View the scheduled assessment on the 'Assess' tab of the experience:
