This article explains how to customize forms used for creating learning experiences and components, like journals, logs, and evidence forms. The four required prompts are discussed in detail in this article on forms. They are shown here for reference only. General Information about Prompts

Each prompt contains a title, description, and checkbox for designating it required. You have the option to make additional prompts required.

In editing mode, most prompts look alike, and do not particularly describe their purpose. Prompts that include adding options (like radio buttons and checkboxes) will also contain an 'Option Text' field and an 'Add Option Button.' 

Each prompt has a unique look and function when added to the form, when selected by the experience creator, and when appearing in the assigned experience plan. The first time you add a prompt, you will not know what the end result looks like, so these articles are designed to show you each stage.

Prompts below marked with an asterisk (*) can only be used once per form. All others can be used multiple times.

 

Required Prompts

Recall from this article that when creating a form, required prompts will appear automatically - you don't need to add them from the menu. Required prompts cannot be removed from your form, but you can edit any of them to change the wording, as well as reorder them by moving them up and down within your form. These and some other prompts marked with an asterisk (*) in this article can only be used once per form.

 

 

Basic Prompts

The designation "basic" is somewhat arbitrary, but it refers generally to the amount of "moving parts"  the prompt contains, or relationships with other Foundry features. All basic prompts are essentially self-contained.

 

Integer

This prompt is designed to hold integers only. It will not save a fraction or a decimal.

Small Text Box

Large Text Box

Formatted Text Box

This is the best option for long blocks of text requiring organization or emphasis.

Note: At present, our developers are working on issues with the Headings and Font Size aspects of the formatted text box. All other features shown are working as expected.

Date

This is a separate feature from the experience start/end date, meant to allow the experience designer to add multiple deadlines to and experience.

Credit Estimate*

As stated in the description box in the images below, the credit estimate feature is for informational purposes only. Foundry does not automatically award credit for completing the experience, and the credit estimate field is not required in order to award credit. It exists separately to hold non-integer decimals.

Radio Button

This allows the experience designer to indicate one of a range of choices. For example, a student requesting the experience may indicate if they want an individual or group experience.  Or, they may indicate what type of result they intend to produce - essay, demonstration, experimental results, etc.

Note: When present in the Experience Plan form, radio buttons and checkboxes (shown below), are selected by the experience designer (whether a student or advisor), not the recipient (student).

When present in Journal, Learning Log, or Evidence forms, they are selected by the recipient (student).

Checkbox

List of items

This is a dynamic feature, completed by the experience designer. There is no need to set the number of list items on the form in advance - each time the designer hits the 'enter' key, another list item will appear.
